Register your name, email address, sex and the month you'd like your email reminder sent. In the month you choose, we will send an e-mail to the address you give us and remind you to call your health care provider to schedule your exam(s).

The Cancer Center of Santa Barbara is attempting to help you remember to have a screening test for colon cancer. However, for a number of reasons, you may not receive a reminder. The sole responsibility to schedule and have an examination rests with you. The Cancer Center disclaims any duty to provide you with any reminder.
The Cancer Center reserves the right to terminate this program at any time without notice. Moreover, the Cancer Center may exclude from the program anyone who it believes may be abusing the program. The Cancer Center has no obligation to send you any reminder or to make sure you have made an appointment.
