Assessment & Home Evaluation
Family Service Agency Senior Outreach Program - (805) 965-1001
Provides bilingual in-home assessment by professional counselor or social worker to identify needs of persons over sixty years of age in Santa Barbara.
Geriatric Assessment Program (GAP) - (805) 346-8391
A program of the Department of Social Services where nurses assess the total situation of individuals on Medi-Cal, sixty years of age or older, who are having trouble coping in their present situation.
Skilled nursing agencies can assess the level of care a person may need when the assessment is ordered by a physician.
